Future student?

We look forward to meeting you! Below are the forms that you will complete prior to your first lesson. If you have any questions, do not hesitate to give us a call!

New Student Forms and Information:

Whether you plan to take group classes or private instruction, we ask that all of our students fill out a 'New Student Registration Form', so that we may gather your contact information, as well as a few other questions that will help us get to know you better.  You may email the completed form, or print and bring with you on your first lesson.

Many of our students ask what should be worn during a dance lesson and if special dance shoes are needed.  We ask that you dress comfortabley, but there is no specific dress code.

It is not necessary to purchase a pair of dance shoes, although there are great benefits to owning your own dance shoes. We suggest wearing shoes that are comfortable to you, but have little to no tread. Tennis shoes and shoes with no back (flip flops, clogs, mules, etc) are not recommended. Ballroom dance shoes are different and beneficial because they typically have suede soles and a flexible arch. We have a great selection of both women and men's dance shoes available at our studio. Stop by the studio and check them out!

Getting Married?

If you are taking advantage of one of our wedding packages, please fill out the following form and return to us prior to your first scheduled lesson:

We can help make your first dance a beautiful performance that you will never forget!  Wedding dance instruction is designed especially for engaged couples who want a choreographed routine, or just a comfortable, natural look when you dance to your wedding music. Of course, there are some things that can help us make your first dance a picture perfect experience. Filling out the above form will let us know if you have a special song picked out, that way we’ll know what dance matches the song. Be sure to allow yourself enough time to learn, rather than waiting until the last minute, so you will increase your confidence on your special day.

Planning a special Mother/Son or Father/Daughter dance?  We can help with that too! We also ask that you fill out the Wedding Dance Lesson form above.
